Comprehensive Care Visit

• A comprehensive care visit was arranged at HTC for evaluation and development of a treatment plan
• Once evaluated the treatment plan was developed and sent to the nursing case manager to coordinate infusions and with the patient
• The patient will continue to follow-up with the HTC annually
