HTC Assessment and Intervention

• Patient was previously treated outside of the HTC setting but is seeking more aggressive management due to recent knee issues
• A comprehensive care visit was arranged at HTC for evaluation and development of a treatment plan
• Once evaluated by the HTC team a treatment plan was developed with the patient
• The patient will continue to follow-up with the HTC at least once annually
