Specialty Pharmacy Collaboration

• The HTC and the specialty pharmacy collaborated on the following:

  –	Prior authorized the treatment plan with the insurance provider
  –	Arranged the CI Equipped the patient with a CADD pump Provided instructions on how to use it, troubleshooting, etc. Provided further education on monitoring the bag, changing it when necessary (the rate was locked and only accessible to trained personnel) Instructed the patient on how to report daily usage to the pharmacist so the next bag could be delivered
  –	Determined who would provide home services Patient was not technically “homebound,” so he was not eligible for skilled home care HTC/specialty pharmacy needed to determine who would make troubleshooting calls/emergency visits
