Reviewing Concomitant Medications and Selecting a HCV Regimen

 

• As a result of recent diagnosis of MDD and necessity of antiviral therapy, consultation between the HTC hematologist, psychiatrist, and pharmacist led to a review of concomitant medications
  • Omeprazole was lowered to 20 mg daily, rosuvastatin was lowered to 10 mg daily, and the patient was counseled not to use St. John’s Wort for depression until HCV therapy completed
• For HCV, ledipasvir/sofosbuvir was selected and prescribed, with the following information supplied as part of the claim:
  • HCV genotype 1
  • Contraindication/intolerance to interferon due to depressive illness
